Aim: Penile necrosis is a urological disease generally associated with chronic degenerative diseases. In Guatemala there is no publication on the subject which means that the clinical case presented provides important information. Clinical Case Presentation: An elderly patient who uses a condom-type catheter for urinary drainage which, when used improperly, causes prolonged compression of the penis. It produces decrease in blood flow, edema and ecchymosis; subsequently causing necrosis of the penis which is treated surgically with total penectomy and perineal urethrostomy due to the failure of conservative treatment. Discussion: The patients with early diagnosis and timely treatment present adequate post-surgical evolution. Conclusions: Despite being a rare disease, penile necrosis has a high morbidity and mortality, mainly if it is associated with chronic degenerative diseases. Early diagnosis and treatment are fundamental pillars to improve the survival of patients.
