Introduction: Fibrothecoma-type tumors are extremely rare, they represent 4% of gonadal neoplasms, they derive from the tunica albuginea or gonadal stroma and resemble the ovarian stroma, they are considered benign. The treatment will be resection and it will be curative if it is a disease located only in the gonads. Objective: To present the clinical case of a 15-year-old male patient with a fibrothecoma-type sex cord stromal tumor, clinical characteristics, laboratory tests, imaging studies and the management provided. Methods: A patient is evaluated in the outpatient clinic with evidence of testicular tumor, complementary studies are carried out and it is taken to the operating room performing radical orchiectomy, the piece is analyzed histologically, reporting fibrothecoma with tumor-free spermatic cord. Patient evolves adequately, is discharged and continues with follow-up by the urology department. Conclusions: In male patients, fibrothecoma-type neoplasia is considered rare and benign, it can present as a slow-growing tumor or precocious sexual puberty and if diagnosed at an early stage, resection will be the treatment of choice and curative. Few cases have been reported worldwide, so patient follow-up is considered strict.
