Objective: To describe the clinical evolution after treatment of patients with emphysematous pyelonephritis.
Material and method: The sample was separated into two groups which had a satisfactory evolution and unfavorable evolution. The sample was also grouped according to the type of treatment used, using a statistical package SPSS 23.0 with p <0.05.
Type of study: Observational, analytical, historical cohort type, with a universe that matches the sample, 18 patients diagnosed with emphysematous pyelonephritis (95% CI) in the period from March 2014 to March 2017.
Results: Patients with diagnosis of pyelonephritis emphysematoses 13 were female, which represents (72.2%), with an average age of 50 years, the history of diabetes was found in 16 patients (88.8%) and lithiasis in 12 patients (66.6%) of the cases, of all these patients to 2 were treated conservatively (11.1%), 8 with catheter JJ (44.4%), 4 with percutaneous nephrostomy (22.2%) and 4 with simple nephrectomy (22.2%), the mortality was 3 patients (16.6%).
Conclusion: The results obtained show that the higher the radiological category, the treatment that will benefit patients should be aggressive resuscitation, broad spectrum antibiotic management, urinary diversion and nephrectomy in patients that is indicated, limiting conservative treatment to patients well selected, the most important risk factor was decompensated diabetes mellitus, associated with a torpid evolution of patients.
