Abstract

Most commonly, bilateral WT is managed by radical nephrectomy (RN) of the more involved kidney and NSS on the less involved contralateral kidney following neoadjuvant chemotherapy. However, bilateral WT patients have been noted to have a significant risk of ESRD due to perioperative renal insults in addition to tumor recurrence and subsequent nephrectomy. We present a case of BWT, aged 12 months, managed with neoadjuvant chemotherapy followed by bilateral NSS and adjuvant chemotherapy and report the follow-up. In our institution we routinely perform any surgical management of bilateral WT in a multidisciplinary manner, with both a pediatric urologist and a pediatric surgeon participating as co-surgeons. We also actively share all patient care duties and management decisions in conjunction with pediatric oncology.
