Abstract

Purpose: We determined whether increasing the number of cores at first prostate biopsy would improve the cancer detection rate without increasing the detection of clinically insignificant tumors and associated morbidity.

Materials and Methods: From April to September 2016 patients scheduled for prostate biopsy were randomized to 12 or 20 (saturation biopsy) core sampling.

Study inclusion criteria were: 1) Beneficiaries, 2) age 45 to 75 years, 3) abnormal digital rectal examination and/or prostate specific antigen between 3 to 20 ng/ml, 4) no previous biopsy. The primary end point was the cancer detection rate. Endpoints were evaluated clinically insignificant detection of cancer and morbidity.

Results: A total of 158 patients were included in the study, preoperative variables were similar in the two groups of 79 patients each. Cancer was detected in 10 patients (12.7%) in group 1 and in 17 (21.5%) in group 2. More cases of insignificant cancer were detected in group 2 (p not significant). There was no significant difference in morbidity.

Conclusions: The saturation biopsy protocol improves prostate cancer detection without increasing morbidity or insignificant cancer detection, this technique may be considered for patients with suspected prostate cancer undergoing a first biopsy.
