Complete ureteral duplication is a rare urological abnormality. The ectopic ureters occur in approximately 1/19000 live births. The diagnosis of ureteral abnormalities in the elderly is not frequent, and most of these cases are associated with incidental findings in imaging studies. When these patients present symptoms, they are usually intimately related to the location of the implantation site of the ectopic ureter. We present the case of a masculine male of 65 years of age with a history of intermittent lumbar pain of 4 months of evolution associated to incomplete emptying, intermittence, urinary urgency, tenesmus and nocturia. A renal ultrasound is performed that reports: Severe right hydronephrosis. A 50 g adenomatous prostate was found by physical examination of the patient with renal function tests and urine test without alterations. The CT scan shows a double complete collector system with an ectopic ureter to the prostatic utricle, superior non-functional pole. Heminephrectomy was performed with laparoscopic ureterectomy without complications.
