Testicular Cancer of Endodermal Breasts in Infants.

A rare case is present in a male patient aged 1 year and 2 months, who starts with an increase in the volume of the left testicle, which is diagnosed by ultrasound with the presence of left testicular mass and was taken to the operating room for left radical orchiectomy. In later anatomopathological studies, they report testicular neoplasm of endodermal sinuses.
Testicular germ cell neoplasms are derived from primordial germ cells, which in early embryonic life migrate from the primitive central nervous system to its posterior location at the level of the gonads. They are tumors of unknown cause and represent 95% of testicular tumors. These tumors are also known as embryonal carcinoma of the infant or yolk sac, present more frequently in infants and children up to 3 years and much less frequently until age 5, have an incidence of 6 per 100,000 in men and with Predominance in white race (ratio 5: 1). For its diagnosis one must take into account the aspects: clinical, that is realized with the finding of a solid testicle or intrascrotal mass, mainly without presence of pain and when examining of firm consistency, well defined and mobile; Humoral in the accomplishment and determination of serum tests of alpha fetus protein, chorionic gonadotropin hormone and lactate dehydrogenase, extremely important for our prognostic evaluation and response to established treatment; Radiological, the main study of great utility is the ultrasound that allows us to assess dimensions of the testicle and the tumor, as well as characteristics of the contralateral testicle; The tomographic studies of the pelvis and thorax are supportive to classify their staging and evaluate treatment and, finally, histologically, the histological confirmation of the tumor by pathology. The literature reports survival range that is related to age, however there are survival rates of 96% after chemotherapy. These types of germ cell neoplasms are very aggressive and capable of producing extensive metastases, however, due to scientific advances and the evolution in medical treatments the prognosis has improved due to its multidisciplinary management.
