Synchronous Renall Cell Carcinoma and Adrenal Adenoma: Case Report

BACKGROUND: In the setting of presentation of a renal mass, lesions in the adrenal glands are suspected of metastasis until proven otherwise. Synchronous renal cell carcinoma and adrenal adenoma are rare and unfrecuently syndromic; however, it Is important to be aware of its existence to avoid over diagnosing a metastatic disease. OBJECTIVE: Present the case of a patient with synchronous renal cell carcinoma and adrenal adenoma, describing the clinical presentation, diagnosis, treatment, and evolution of this rare pathological association. MATERIALS AND METHODS: A 52 year-old female patient hospitalized for follow up of a left renal mass diagnosed by ultrasound performed because of a diffuse abdominal pain of 3 months evolution. During her hospital stay, a videolaparscopic left radical nephrectomy was performed. The pathology department received a 13 x 8 cms left kidney, whose section showed a mass of 8 x 7 cms in the upper pole and middle third. Left adrenal gland containing a yellowish mass, 2 x 1.5 x 1 cms, was also received. RESULTS: Microscopy of the renal mass revealed a neoplasm composed of cells with nested growth pattern, clear cytoplasm and visible nucleoli; acompanied by a network of small vessels.
