Percutaneous Nephrolithotomy Experience at Medilaser Hospital, Quetzaltenango, Guatemala

Introduction: Percutaneous nephrolithotomy is a minimally invasive procedure for kidney stone removal, Fernstrom and Johansson first reported the technique in 1976. Since then, advances in surgical technique and technology have allowed the urologist to remove calculi percutaneously with increasing efficiency. Multiple techniques have been described to perform this procedure with low morbidity and faster recovery replacing open approach in most hospitals worldwide. Objective: To present the experience in percutaneous nephrolithotomy at Medilaser Hospital in Quetzaltenango, Guatemala. Material and Methods: A descriptive, retrospective study, based on systematic review of clinical records of patients who were diagnosed with renal lithiasis, who underwent percutaneous nephrolithotomy at Medilaser Hospital, between January 2020 until April 2023. Results: A total of 52 Percutaneous Nephrolithotomy procedures were performed, most of them with overweight (mean BMI 27.43 kg/m2), the procedures were carried out through prone in 29 an supine in 23 patients. The stones were 3.02 cm average, lower pole access was achieved in most of the cases, the overall complication rate was 25%. Of these, 6% had major complications, Clavien-Dindo classification was used. Conclusions: Percutaneous Nephrolithotomy is an effective, safe and low morbidity procedure to treat stones of 2 cm and up, we believe the knowledge of different techniques translate on better stone free rates. The modified Clavien- Dindo morbidity score is a reliable tool.
