Experience and Review of the Literature in the Management of Fournier´s Gangrene in a Referral Hospital in Ecuador, 2019-2020.

Fournier's gangrene is considered a polymicrobial and idiopathic necrotizing fasciitis that affects perineal, genital and perianal areas, it has a high morbidity and mortality due to the rapid progression of the process. Objective: To demonstrate pharmacological and surgical treatment, as well as microorganisms found in cases of patients with Fournier's gangrene at the Hospital de Especialidades Eugenio Espejo. Material and Methods: Retrospective observational study. 7 cases of individuals between the years 2019-2020 were analyzed, 3 cases were excluded. Likewise, information on clinical, epidemiological and treatment data was assessed. Results: Of the 7 cases studied, 100% were male, the average age was 56 years, Hypertension and benign prostatic hyperplasia was the main predisposing disease (71%). The average time for surgical intervention was 21.5 hours. All patients were treated with surgical debridement and broad-spectrum antibiotic therapy, all required surgical reintervention. None of the patients presented surgical complications, nor died at 18 months follow-up. Discussion: The purpose of treatment is to reduce systemic toxicity, eliminate the causative microorganisms and control the progression of the infection together with the removal of necrotic tissue. Conclusion: It is essential to establish a diagnosis, as well as timely antibiotic and urgent treatment, the pathology is known for its rapid progression, with a high mortality rate and serious sequelae.
