We report a case of Giant Prostatic Hyperplasia treated by Transvesical Prostatectomy. A 68-year-old man had a history of low obstructive urinary symptoms. A complete abdominal tomography was performed, visualizing prostate with a calculated weight greater than 500 gr. The value of the prostate specific antigen was at 4,021 ng / ml. We perform open Prostatectomy through the bladder. The adenoma was removed in one piece, weighing 470 grams. Histopathologic examination revealed Glandular Prostatic Nodular Hyperplasia. Open surgery is the recommended treatment for this type of prostate pathology, however new surgical options, such as endoscopic enucleation, may be feasible for patients in the future.
